Flu Shots
For the eighth consecutive year the Yankton School District and Avera Sacred Heart Hospital is offering free Seasonal Flu vaccines for children in grades JrK-12th grade.
Vaccinating children helps protect them from serious influenza illness and complications and decreases the spread of influenza in the community, as children are often the biggest spreaders of influenza to adults and other children.
If this is the first time your child has received a flu shot and is under the age of 9, please contact your healthcare provider if a second dose is needed.

If you would like your child to receive the vaccine, please complete the following:

Review the Vaccine Information Statement carefully – keep for future reference.

Complete the 2014-2015 Influenza Consent Form.

Return the completed consent form to the school your child/children attend or to their elementary teacher by the dates listed on the back of this sheet.

ELEMENTARY SCHOOL STUDENTS

A free Seasonal Flu clinic will be held for children in Jr. Kindergarten through 5th grade at each elementary school.  Parents need not be present but the attached consent form must be signed by a parent for all students wishing to get the vaccine in school. Please return these forms to your child’s teacher or the school office by Tuesday, October 28.  Below is the schedule for each elementary school

Lincoln School Monday, November 3
Stewart School Monday, November 3
Webster School Thursday, November 6
Beadle School Thursday, November 6

MIDDLE SCHOOL STUDENTS

A free Seasonal Flu clinic will be held for the middle school students at the Yankton Middle School on Monday, October 27.  Parents need not be present but the attached consent form must be signed by a parent for all students wishing to get the vaccine in school.  Please return the consent form to the office by Tuesday,
October 21.

HIGH SCHOOL STUDENTS

A free Seasonal Flu clinic for all high school students will be held in the morning at the Yankton High School on Monday, October 27.  Parents need not be present but the attached consent form must be signed by a parent for all students under the age of 18.  Students age 18 and older may sign their own forms.  Please return the form to the office by Tuesday, October 21.
